Treatment doesn’t end once a patient leaves a mental health facility – they are entitled to continued mental health care. Before discharge, the treatment team considers the patient’s safety when they return home. The treatment plan — which includes discharge planning — connects patients with community-based services, family and friends, and other resources to help ensure continued success. Patients should participate as much as possible in the discharge planning process.
