You are a vital partner in helping to ensure the ward receives the best possible treatment. One important way you can get involved is helping with discharge planning when the ward enters the facility. Not only does it help ensure that the ward gets the best possible care, but it helps make your life easier because you can begin making personal plans and arrangements early.

Those plans might involve living arrangements or transportation to doctor’s appointments for continued care.
