Ongoing Duties: Complete and File an Annual Report

Every year as a guardian, you must complete a report. Download the annual report form. Or pick one up from the office of the clerk of district court.

Who Receives the Annual Report?

You must file the annual report with the clerk of district court by mail or email.

Interested Parties

Mail a copy of the report to the interested persons designated in the Findings of Fact and Order Appointing Guardian document you received from the court when the guardianship was ordered by the court.

The Ward

You must mail a copy of the annual report to the ward. The Ward’s copy must include the statement below and cannot be less than double-spaced and 12-point type.

You, as ward, have the right to petition the court to change, limit, or end this guardianship at any time. Any person who knowingly interferes with your request to the court or judge may be found guilty of contempt of court.

What should the annual report say?

The annual report must detail these nine areas.

  1. A summary of the ward’s current physical and mental condition
  2. Any time you exercised your legal authority as guardian
  3. Changes in where the ward lives and the reason for moving
  4. A list of medical exams or treatments received or withheld
  5. Any other services the ward received
  6. The ward’s income, assets, and expenses
  7. All sales or transfers of the ward’s property and assets
  8. Whether the ward still needs a guardian
  9. Whether your powers of authority should increase, be limited, or be removed

Any decision making authority you exercise as guardian must be reported on the annual report.
